2026 Registration Refund Request Form

Please fill out this form to request a registration refund. Refunds will only be accepted through this form.

CMAA REFUND DECLARATION FORM

If you are requesting a refund for your child's/children's registration for the 2026 season we ask that you please fill out this form.

Reminder of refund details that you saw in your registration process.

Refund Policy:

Refunds:
If your child requires a refund prior to February 1 you will receive a full (100%) refund minus a $5.00 processing fee

If your child requires a refund after uniforms are ordered on February 2 to May 13 you will receive 30% of the registration fee minus a $5.00 processing fee.

After May 13th, refunds will be discussed on an individual basis.

Refunds must be accompanied by a Request for Refund Form (found on the website), a $5.00 processing fee will be charged and all refunds will be made through Sportspay.

We will reverse your payment through Sports Pay or E-transfer.
